2025-26 GHSA PHYSICAL EXAMINATION PACKET

Lassiter Basketball is more than a team—it’s a tradition of excellence, grit, and teamwork. Whether you're honing your skills or pushing the limits of competition, we play with heart and determination. Join us as we build champions on and off the court.

Ready to be part of the action? If you’re interested in trying out next season, complete the GHSA Physical Examination Packet and upload it to ParentVue’s Athletic Registration before participating.

Parent/guardian and student signatures are required.

The Athletic Registration User Guide for Parents is a step-by-step manual from the Cobb County School District designed to help parents register their students (grades 8–12) for school athletics through ParentVUE. It walks parents through logging in, selecting the correct school and sports, and completing essential sections: consent forms, insurance details, medical history, and document uploads (like physicals).
